Applicants Must:
- Be a Michigan Resident
- Be between the ages of 6–14
- Have a parent or guardian complete the application
Priority consideration may be given to families with modest annual income.

Before you begin, have the following ready:
- Parent/Guardian ready to complete the application
- Photo of Michigan State ID
- Youth Report Card or Alternate Supporting Document (see FAQ for options)
